Biologic's Registry / Forms

The SARAA biologic guidelines state that All rheumatological patients who wish to use biologic therapies must be registered with the SARAA Biologics Registry.

Any changes in biologic therapy, adverse events during the course of treatment and 6 monthly clinical updates are to be added to the registry.

SARAA members may enter information into the registry by logging into the secure section of this website, or by downloading and completing the forms below. After completing the required information - fax the form to Fax: (011) 888 6958.

The SARAA administrator will transfer the information to the online database where after a request will be sent to the SARAA biologic panel to authorize or reject the submission.

Authorized applications are returned to the relevant medical practitioner.

• Patient consent - click here to download [PDF} the patient consent form - This must be completed before a patient's information is captured into the registry. The practitioner Is required to keep the patient consent form in the patient’s file. DO NOT SEND patient consent forms to the registry.

• Doctor Consent - click here to download[PDF] the Doctor consent form - This must be completed by the Doctor before a patient's information is captured into the registry. The practitioner Is required to forward this form to the system administrator - or go online to accept the Biologic Registry terms of use.
